Appointments

Sessions are by appointment only. Initial appointments are 60 minutes, with follow ups being 30 minutes. We offer Telehealth services through a secure, HIPAA-compliant platform. All intake forms must be completed prior to the first appointment.

Cancellations and No Shows

If you need to cancel or reschedule, please notify us at least 24 hours in advance. Late cancellations or missed appointments will result in a $75 fee.

Communication

For scheduling or routine messages, please use the secure patient portal or reach out via email. If you are experiencing a medical or psychiatric emergency, please call 911 or go to the nearest emergency room.

Fees & Insurance

We are in network with Husky, United Health Care, Aetna, and Cigna. We also offer self pay. Please reach out to your insurance provider to get coverage details. We require a credit card on file which will be used as a method of payment for balances and charges for which your insurance company considers your financial responsibility.

Forms and Paperwork

Requests for administrative paperwork such as FMLA forms, disability documentation, or accommodation letters are not billable to insurance and will incur an additional fee. Please allow 5-10 business days for completion. FMLA or disability forms: $75 | Accommodation letters: $50

Prescription Refills

Please fill out the 'Prescription Request' form on the homepage of our website for a refill request. Please request medication refills at least 3 business days in advance. We do not process refill requests after hours, on weekends, or during holidays.
