Out-of-Network (OON) Options

We can provide a superbill for submission for out-of-network reimbursement

OON Fee Schedule

New Patient Evaluation: $300 (75 minutes)

Standard follow-up: $150 (30 minutes)

At patient request we offer shortened or extended follow-up appointments at different rates. Shortened 15 minute follow-up (reserved for maintenance medication check) is billed at $100. Extended 45 minute follow-up appointment is billed at $225. Exceeding allotted time during you appointment will be charged for an additional 15 minute block.

*CPT coding for insurance claims can be provided at patient request
